Heinrich's theory also estimated that 88% of all accidents result from people's decisions to perform an unsafe action.

Criticism of The Safety Triangle

These values have since been shown only to be accurate when applied to large datasets and a wide range of activities. For example, high-risk activities like confined space entry will have closer ratios of serious, minor, and near-miss incidents.

Heinrich's theory also blames human action for most workplace accidents without considering how deficient management systems can cause many accidents. Critics have also targeted the triangle's focus on reducing minor accidents and treating all near misses and minor incidents as if they carry the same level of risk. This approach has prompted workplace supervisors to overlook more severe but less probable potential Serious Injuries and Fatalities (SIF) in work planning, concentrating instead on reducing more common but less severe risks.

Critics also claim the Safety Triangle fails to account for the full extent of costs resulting from serious incidents and lost-time injuries. Additionally, it underestimates the potential for hidden or indirect costs, such as reputational damage, decreased employee morale, and regulatory fines, which may not be immediately apparent but can have a substantial impact on an organization. Many safety professionals argue that a more comprehensive approach is necessary to fully understand the complex relationship between incident frequency and the true costs associated with them.
